Please fill out the following form for each student who wishes to participate in the MOH Art Club. Please note, this club is organized and run by MOH PTO Parent volunteers.
This club will run once a month from 3:30-4:45pm on the following dates:
2/12, 3/12, 4/9, 5/14, 6/11
Students must be picked up no later than 4:45pm from the front vestibule. Parent Volunteer contact information will be shared once your student is officially registered.
Registration closes on 1/31, no late registration will be accepted.
